ABSTRACT

Development of extramural health care for chronic wounds is still in its infancy in China, and thus it is urgent and vital to establish a correct concept and practicable principles. The authors reviewed recent domestic and international literature and summarized the following treatment procedures and principles for extramural health care of chronic wounds. (1) The patient needs to do self-assessment of the wound by using available simple methods; (2) The patient consults with professional physicians or nurses on wound care to define the severity and etiology of the non-healing wound; (3) Professionals evaluate the existing treatment strategies; (4) Etiological treatments are given by professionals; (5) Patients buy needed dressings via the more convenient ways from pharmacies, e-commerce platform or others; (6) Professionals provide a standardized and reasonable therapeutic plan based on the patient's wound conditions; (7) Both professionals and the patient pay attention to complications to prevent adverse outcomes; (8) Professionals strengthen the public education on wound care and integrated rehabilitation. This review expected to provide new perspectives on the therapeutic strategies for chronic wounds in an extramural setting.

ABSTRACT

Este trabalho teve como objetivo realizar uma revisão integrativa sobre a Ozonioterapia como prática complementar no tratamento de lesões em seres humanos. Efetuou-se busca sistematizada nos portais: Biblioteca virtual em saúde (BVS), Biblioteca virtual em saúde de medicinas tradicionais complementares e integrativas (BVS MTCI) e Business source complete (EBSCO) nas seguintes bases de dados: Literatura latino-americana e do caribe em ciências da saúde (LILACS), Base de dados de Enfermagem (BDENF) e Medical literature analysis and retrievel system Online (MEDLINE), com cruzamento dos descritores: ozônio; ferimentos; lesões; cicatrização de feridas. Após os critérios de inclusão serem aplicados, observou-se que dez artigos atingiram todos os critérios previamente definidos. Os estudos apontam que a Ozonioterapia apresenta resultados positivos como opção na terapêutica complementar de lesões. Porém, é um recurso terapêutico com baixa adesão. Faz se necessário maior empenho cientifico sobre a temática. PALAVRAS-CHAVE: Ozônio; Ferimentos; Lesões; Cicatrização de feridas.


Currently, there are several treatments as well as ozone therapy that aim to improve wounds. This work aimed to carry out an integrative review on Ozone Therapy as a complementary practice in the treatment of injuries in humans. A systematic search was conducted on the portals: Virtual Health Library (VHL), Virtual Health Library of Complementary and Integrative Traditional Medicines (VHL MTCI) and Business Source Complete (EBSCO) in the following databases: Latin American and Caribbean Literature in Health Science (LILACS) , Nursing Database (BDENF) and Online Medical Literature Review and Retrieval System (MEDLINE), by crossing descriptors: Ozone, Wounds and Injuries, Healing injuries. After the inclusion criteria were applied, it was observed that ten articles reached all the criteria previously defined. The studies indicate that Ozone therapy presents positive results as a complementary therapy option on wounds. However, it is a therapeutical resource with low adherence. It is necessary a greater scientific commitment on the theme.


Este trabajo tuvo como objetivo realizar una revisión integradora sobre la ozonoterapia como práctica complementaria en el tratamiento de lesiones en humanos. Se realizó una búsqueda sistemática en los siguientes portales: Biblioteca Virtual de Salud (BVS), Biblioteca Virtual de Salud de Medicinas Tradicionales, Complementarias e Integrativas (BVS MTCI) y Business Source Complete (EBSCO) en las siguientes bases de datos: Latin American and Caribbean Health Sciences Literature (LILACS), Nursing Database (BDENF) y Medical Literature Analysis and Retrieval System Online (MEDLINE), utilizando las palabras clave: ozono; heridas; lesiones; curación de heridas. Una vez aplicados los criterios de inclusión, se observó que diez artículos cumplían todos los criterios previamente definidos. Los estudios demostraron que la ozonoterapia tiene resultados positivos como opción terapéutica complementaria para las lesiones. Sin embargo, es un recurso terapéutico con un bajo cumplimiento. Es necesario un mayor esfuerzo científico sobre el tema.

ABSTRACT Introduction: Hydrokinesitherapy is a rehabilitation method used for the treatment of different types of orthopedic, neurological, respiratory problems, etc. However, the existing literature presents not so much evidence regarding the greater efficacy of water treatment compared to dry treatment in patients with post-traumatic outcomes. Objective: Through the administration of a questionnaire, we tried to investigate the rehabilitation experience of hydrokinetic therapists (trained with the so-called Sequential and Preparatory Approach) in order to understand hydrokinesitherapy areas and methods of proper application based on experts' opinion. Methods: A cross-sectional study was carried out in 2020 by administering a questionnaire to physiotherapists trained in hydrokinesitherapy according to the Sequential and Preparatory Approach method, with the aim to investigate their opinion on the use of hydrokinesitherapy in patients with various orthopedic-traumatological problems. Microsoft FORMS® platform was used for the administration of the questionnaire. Results: Sixty-two users participated in the study. From their answers emerged that the use of hydrokinesitherapy is not recommended for the treatment of post-traumatic hypersensitization of peri-lesional tissues (hypersensitive scars; general hypersensitization) and neuropathic syndromes (of the upper and lower limbs). Conclusions: The data obtained from administering the questionnaire were not sufficient to create a specific path to define the appropriateness of hydrokinesitherapy for some orthopedic-traumatological problems but could be considered a good starting point on which to build future developments through further studies.


RESUMEN Introducción: La terapia hidrocinética es un método rehabilitador utilizado para el tratamiento de diferentes tipos de problemas ortopédicos, neurológicos, respiratorios, entre otros. Sin embargo, en la literatura existente no abundan evidencias en cuanto a la mayor eficacia del tratamiento con agua en comparación con el tratamiento seco en pacientes con resultados traumáticos. Objetivo: A través de un cuestionario, se intentó investigar la experiencia de rehabilitación de los terapeutas hidrocinéticos (entrenados con el llamado Enfoque Secuencial y Preparatorio) para comprender las áreas de la terapia hidrocinética y los métodos de aplicación adecuados con base en la opinión de expertos. Métodos: Se realizó un estudio transversal en el año 2020 mediante la administración de un cuestionario a fisioterapeutas formados en la terapia hidrocinética según el método de Abordaje Secuencial y Preparatorio, con el objetivo de averiguar su opinión sobre el uso de la terapia hidrocinética en pacientes con diversos problemas ortopédico-traumatológicos. Se utilizó la plataforma Microsoft FORMS® para la administración del cuestionario. Resultados: Sesenta y dos usuarios participaron en el estudio. De sus respuestas surgió que no se recomienda el uso de la terapia hidrocinética para el tratamiento de la hipersensibilización postraumática de los tejidos perilesionales (cicatrices hipersensibles; hipersensibilización general) y síndromes neuropáticos (de miembros superiores e inferiores). Conclusiones: Los datos obtenidos del cuestionario no fueron suficientes para crear una ruta específica que defina la idoneidad de la terapia hidrocinética para algunos problemas ortopédicos-traumatológicos, pero podría considerarse un buen punto de partida para construir futuros desarrollos a través de estudios adicionales.

ABSTRACT

Las lesiones de punta de dedo son comunes en pacientes pediátricos que acuden a salas de Emergencia; son causadas por accidentes en el hogar y con aplastamiento en puerta como principal mecanismo de trauma. El tratamiento se realiza individualizado por cirujanos plásticos y cirujanos pediátricos en dependencia de la herida y el compromiso tisular. El propósito de este estudio es describir el manejo de pacientes con lesiones de punta de dedo en un centro pediátrico. Se realizó un análisis de historias clínicas en el período entre enero 2015 y diciembre 2020, se evaluaron 228 pacientes atendidos en el Hospital de Niños Dr. Roberto Gilbert Elizalde, en Guayaquil, Ecuador. Los resultados incluyen un predominio de pacientes en edad preescolar (67%), masculinos (56%) y de mano derecha (53%). Los dedos más afectados fueron el medio y anular. Los procedimientos fueron realizados por cirujanos plásticos (55%) y la técnica electiva fue bajo anestesia general. La extensión del daño involucró tejidos blandos (92%), avulsión de uña (29%) y fracturas asociadas (15%). El 3% de los pacientes presentó complicaciones que requirieron seguimiento. El tipo de reparo se seleccionó en dependencia de la extensión de la herida, se realizaron suturas simples (37%) y suturas complejas con reparo de la uña (27%). Nuestros datos son similares a los descritos en la literatura internacional, con buenos resultados a largo plazo, menor tasa de complicaciones y respuestas similares en los reparos realizados por cirujanos plásticos y cirujanos pediatras.


Fingertip injuries are common in pediatric patients who present to the emergency room. They are usually caused by accidents at home, being door-crush the main mechanism. Treatment is performed on an individual basis, by a Plastic or General Pediatric Surgeon, according to the wound findings and tissue involvement. The purpose of this study is to describe the management on a pediatric center in patients with a fingertip wound. A descriptive and retrospective analysis of medical records was carried out from January 2015 until December 2020. In this period 228 patients were seen in the Dr. Roberto Gilbert Elizalde Children´s Hospital in Guayaquil, Ecuador. A high incidence was found in the preschool age with 67% of cases. The injuries were more common in males (56 %) and more predominant on the right hand (53%). The most affected fingers were the middle and annular. Procedures were mostly performed by Pediatric Surgeons (55%) and the elective technique was under general anesthesia. Damage extension of the fingertip included soft tissues in 92% of patients, nail avulsion in 29% and associated fractures in 15%. Only 3% of patients developed complications that required follow up. The type of repair was selected according to the extension of the injury. Mostly simple sutures (37%) and complex sutures with nail repair (27%)were used.Our data is similar to the one found in international literature, with good long-term outcomes, minor complications and similar results when the repair was performed by a Plastic or Pediatric surgeon

O objetivo do presente trabalho foi relatar a eficácia da fotobiomodulação laser na abordagem de úlceras venosas de membros inferiores e como se dá o papel da enfermagem nesse contexto. Tratou-se de uma revisão sistemática que utilizou as bases de dados eletrônicas PubMed, BVS, SCIELO e Lilacs, além da busca no repositório de teses e dissertações pelo cruzamento dos descritores livres e provenientes do DeCS através dos operadores booleanos AND e OR: "laserterapia", "fotobiomodulação laser" e "úlceras venosas". Os critérios de inclusão estabelecidos foram estudos prospectivos, teses e dissertações escritas nos idiomas inglês, português e espanhol, e publicações entre o período de 2010-2020. A revisão foi conduzida conforme metodologia PRISMA, utilizou-se os critérios de Jadad para avaliação da qualidade dos manuscritos. Dos 104 artigos encontrados, apenas 8 contemplavam os critérios de inclusão, tendo uma população total de 942 pacientes. Notou-se que a maioria dos estudos mostrou a eficácia do uso da laserterapia, principalmente quando associado ao uso da terapêutica comum, como a terapia compressiva, agentes tópicos e farmacoterapia. A fotobiomodulação laser demonstrou ser eficaz, porém ainda são necessários mais estudos com o objetivo de uniformizar os parâmetros de aplicação dessa modalidade terapêutica.


The objective of this work was to investigate the effectiveness of laser photo-biomodulation in lower limb venous ulcers and the role of nursing in this context. This is a systematic review, using the PubMed, BVS, SCIELO and Lilacs electronic databases in addition to searching the repository of thesis and dissertations by crossing the free descriptors coming from the DeCS through the Boolean operators AND and OR: "laser therapy", "laser photo-biomodulation" and "venous ulcers". The inclusion criteria were prospective studies, theses and dissertations written in English, Portuguese, or Spanish, and publications in the 2010-2020 period. From the 104 articles found, only 8 met the inclusion criteria, with a total population of 942 patients. It could be observed that most studies showed the effectiveness of using laser therapy, especially when associated with the use of common therapy, such as the compressive therapy, topical agents, and pharmacotherapy. The laser photo-biomodulation was also shown as being effective, but further studies are needed to standardize the application parameters for this therapeutic modality.

RESUMEN: Las lesiones iatrogénicas de las vías biliares (LIVB), en el curso de una colecistectomía laparoscópica (CL), son complicaciones que causan resultados inesperados para cirujanos un incremento en los riesgos de los pacientes (morbilidad y mortalidad), afectando su calidad de vida. Asimismo, causan situaciones difíciles desde el punto de vista técnico para el cirujano que debe repararlas desde un punto de vista técnico. El objetivo de este manuscrito fue resumir la información referente a las LIVB y describir su morfología y opciones diagnóstico-terapéuticas.


SUMMARY: Iatrogenic Bile duct injuries (IBDI), during laparoscopic cholecystectomy (CL), are complications that cause unexpected results for surgeons, an increment in patient risks (morbidity and mortality), and affect the patient´s quality of life. At the same time, they create difficult situations for the repairing surgeon from a technical point of view. The aim of this manuscript was to summarize the information regarding IBDI and to describe its morphology and diagnostic-therapeutic options.

Purpose: Negative pressure wound therapy (NPWT) has revolutionized wound care, but its high cost reduces the procedure's availability. To solve the problem, streamlined vacuum dressings systems have been proposed, but the utility of these devices has been poorly studied. The objective of this study was to evaluate a simplified vacuum dressing system model (SVDM). Methods: Randomized clinical trial in which wounds were treated with SVDM compared to a complex occlusive dressing (silver hydrofiber, SHF). The analyzed outcomes were cleaning, presence of granulation tissue, clinical appearance, and indication for surgical closure of wounds. Results: Fifty injuries were treated (25 in each group), most located on lower limbs. SVDM proved to be more effective than SHF in the evaluated outcomes. Wound recalcitrance reduced the effectiveness of the equipment used. Despite its efficacy, complications occurred, the most frequent related to dressing changes: minor bleeding, foam adherence to a wound bed, and pain. Only for bleeding no favorable risk-benefit ratio was found. There were no severe complications, worsening conditions of injuries, or deaths. Conclusions: SVDM proved to be an effective and acceptably safe device for managing studied wounds.

Resumo Objetivo Identificar intervenções baseadas em evidências científicas eficazes para o tratamento de trauma mamilar decorrente da amamentação. Métodos Revisão sistemática realizada nas bases de dados MEDLINE®/PubMed, Scopus, Cinahl, Web of Science, Cochrane e Lilacs, entre agosto e setembro de 2020. Foram utilizados os descritores nipple* AND wound and injuries OR trauma AND therapeutic OR treatment AND breastfeeding (Mamilo* e ferida e lesões ou trauma e terapêutica ou tratamento e amamentação). Os critérios de elegibilidade foram: estudos de intervenção, sem restrição de idioma e tempo, texto completo e que não abordassem trauma mamilar não relacionado à amamentação. Foram consideradas intervenções eficazes aquelas com desfecho positivo para reparação tecidual e/ou dor. As buscas foram em pares, e a qualidade dos ensaios foi avaliada pela escala Jadad e pelo Grading of Recommendations Assessment, Development and Evaluation. Resultados A amostra final foi de sete artigos, encontrados nas bases de dados Cinahl, Web of Science e Scopus. As intervenções eficazes encontradas envolveram: pomada de camomila, uso de lanolina altamente purificada, fotobiomodulação com laser de baixa potência, mel, mil-folhas e leite materno. Cada intervenção encontrada foi usada em situações e de maneiras diferentes, que devem ser consideradas para a prática clínica. Orientações sobre a pega estiveram presentes em associação com a maioria das intervenções eficazes. Conclusão As intervenções evidenciadas podem contribuir para diminuir as dificuldades na amamentação, na redução da dor e na reparação tecidual das lesões.

Se consideraron intervenciones eficaces las que presentaron desenlace positivo para la reparación de los tejidos y del dolor. Las búsquedas se hicieron por pares y la calidad de los ensayos se evaluó a través de la escala Jadad y por el Grading of Recommendations Assessment, Development and Evaluation. Resultados La muestra final estuvo formada por siete artículos, encontrados en las bases de datos Cinahl, Web of Science y Scopus. Las intervenciones eficaces encontradas contuvieron: pomada de camomila, uso de lanolina altamente purificada, fotobiomodulación con láser de baja potencia, miel, milenrama y leche materna. Cada intervención encontrada fue usada en situaciones y de maneras distintas, que deben ser consideradas para la práctica clínica. Orientaciones sobre la prendida estuvieron presentes junto con la mayoría de las intervenciones eficaces. Conclusión Las intervenciones evidenciadas pueden contribuir para reducir las dificultades de la lactancia y el dolor, y para reparar los tejidos de las lesiones.


Abstract Objective To identify interventions based on effective scientific evidence for treating nipple trauma due to breastfeeding. Methods This is a systematic review, carried out in the MEDLINE®/PubMed®, Scopus, CINAHL, Web of Science, Cochrane and LILACS databases, between August and September 2020. The descriptors nipple* AND wound and injuries OR trauma AND therapeutic OR treatment AND breastfeeding were used. Intervention studies, without language and time restriction, full text and that did not address nipple trauma not related to breastfeeding were included. Effective interventions were considered those with positive outcome for tissue repair and/or pain. The searches were in pairs, and the quality of the trials was assessed by the Jadad scale and by the Grading of Recommendations Assessment, Development and Evaluation. Results The final sample was composed of seven articles, found in the CINAHL, Web of Science and Scopus databases. The effective interventions found involved chamomile ointment, use of highly purified lanolin, low-level laser photobiomodulation, honey, millefeuille and breast milk. Each intervention found was used in different situations and in different ways, which should be considered for clinical practice. Guidance on the handle was present in association with most effective interventions. Conclusion The interventions evidenced may contribute to reduce difficulties in breastfeeding, in the reduction of pain and injury tissue repair.

Resumo Objetivo: Desenvolver e validar um curso de educação continuada em enfermagem a distância para tratamento de feridas em prisões. Métodos: Estudo descritivo sobre o desenvolvimento, a configuração e a validação de um curso on-line para tratamento de feridas, destinado a profissionais de saúde do sistema prisional e realizado em três etapas. A primeira fase foi a busca de anterioridade, para verificar a existência de cursos de tratamento em feridas voltado ao sistema prisional e elaborar o conteúdo do curso, com base em um protocolo de tratamento de feridas e em artigos de bases de dados. A segunda fase consistiu na configuração do curso na plataforma Moodle. Na terceira fase, foram aplicadas duas rodadas da técnica Delphi para validação do curso, com consulta de sete especialistas convidados com titulação mínima de especialistas em tratamento de feridas (estomaterapia) ou com pelo menos um ano de experiência em atuação na área de cuidado com feridas. Resultados: O curso de tratamento de feridas foi configurado na plataforma Moodle, com seis módulos, seis exercícios e uma avaliação de aprendizagem. A validação do conteúdo do curso ocorreu após a realização de duas rodadas de avaliação dos juízes, com Índice de Validade de Conteúdo de 0,83 na primeira e 0,98 na segunda. Conclusão: Foi desenvolvido e validado o curso de educação continuada em enfermagem a distância para tratamento de feridas em prisões, denominado "Educação Permanente On-line sobre Tratamento de Feridas".


Resumen Objetivo: Desarrollar y validar un curso de educación continua en enfermería a distancia para el tratamiento de heridas en prisiones. Métodos: Estudio descriptivo sobre el desarrollo, la configuración y la validación de un curso en línea para el tratamiento de heridas, destinado a profesionales de salud del sistema penitenciario y realizado en tres etapas. La primera fase fue la búsqueda de la anterioridad, para la verificación de la existencia de cursos de tratamiento de heridas direccionados al sistema penitenciario y elaborar el contenido del curso con base a un protocolo de tratamiento de heridas y a artículos de bases de datos. La segunda fase consistió en la configuración del curso en la plataforma Moodle. En la tercera fase, se aplicaron dos rondas del método Delphi para la validación del curso, con la consulta a siete especialistas invitados con titulación mínima de especialistas en el tratamiento de heridas (estomaterapia) o con un mínimo de un año de experiencia en la actuación en el área de cuidado con heridas. Resultados: El curso de tratamiento de heridas se configuró en la plataforma Moodle, con seis módulos, seis ejercicios y una evaluación de aprendizaje. La validación del contenido del curso se dio después de la realización de dos rondas de evaluación de los jueces, con Índice de Validez de Contenido de 0,83 en la primera y de 0,98 en la segunda. Conclusión: Se desarrolló y validó el curso de educación continua en enfermería a distancia para el tratamiento de heridas en prisiones, denominado "Educación Permanente en línea sobre el Tratamiento de Heridas" (Educação Permanente On-line sobre Tratamento de Feridas).


Abstract Objective: To develop and validate a distance continuing education course in nursing for wound care in prisons. Methods: Descriptive study on the development, configuration and validation of an online course in wound care aimed at health professionals in the prison system performed in three steps. The first step was the search for anteriority to check the existence of courses of wound care aimed at the prison system and prepare the course content based on a wound care protocol and on articles from databases. The second step consisted of setting up the course on the Moodle platform. In the third step, two rounds of the Delphi technique were applied to validate the course, including the consultation of seven invited experts with a minimum qualification of specialists in wound care (stomatherapy) or with at least one year of experience in the area of wound care. Results: The wound care course was set up on the Moodle platform with six modules, six exercises and a learning assessment. The course was validated after two rounds of evaluation by the judges with a Content Validity Index of 0.83 in the first round and 0.98 in the second. Conclusion: The distance continuing education course in nursing for wound care in prisons called "Online Continuing Education in Wound Care" was developed and validated.

RESUMO Objetivo identificar na literatura a formação do biofilme e o seu comportamento diante das intervenções em feridas cutâneas. Métodos revisão integrativa, realizada nas bases de dados Cumulative Index to Nursing and Allied Health Literature , Literatura Latino-Americana e do Caribe em Ciências da Saúde, EMBASE, Scopus, The Cochrane Library Collaboration , MEDLINE/PubMed e Science Direct, sem delimitação temporal. Foram selecionados 19 estudos. Avaliação das informações ocorreu de forma descritiva, confrontando com os achados pertinentes. Resultados os estudos da amostra foram publicados no idioma inglês e contemplaram três tipos de pesquisa de biofilme: dois clínicos, seis in vitro e 11 in vivo (animal). Incluíram-se três temas: criação de modelo biofilme (n=4), avaliação do biofilme (n=3), comportamento do biofilme diante de intervenções para o seu manejo (n=12). Conclusão efeitos prejudiciais do biofilme na cicatrização de feridas foram confirmados. Diversas intervenções foram capazes de reduzir e eliminar o biofilme nos modelos in vitro e in vivo . Contribuições para a prática constatou-se que avaliação clínica da lesão não permite identificar o biofilme, inclusive quando presente encontra-se abaixo da superfície da lesão. Este achado suscita reflexão por parte dos enfermeiros a respeito das intervenções adotadas para a remoção do biofilme.


ABSTRACT Objective to identify in the literature the biofilm formation and its behavior when faced with interventions in cutaneous wounds. Methods an integrative review, carried out in the Cumulative Index to Nursing and Allied Health Literature, Latin American and Caribbean Health Sciences Literature, EMBASE, Scopus, The Cochrane Library Collaboration, MEDLINE/PubMed and Science Direct databases, without temporal delimitation. Nineteen studies were selected. The information was evaluated descriptively, comparing it with the pertinent findings. Results the sample studies were published in English and included three types of biofilm research: two clinical, six in vitro and 11 in vivo (animal). Three themes were included: biofilm model creation (n=4), biofilm assessment (n=3), biofilm behavior before interventions for its management (n=12). Conclusion the detrimental effects of biofilm on wound healing have been confirmed. Several interventions were able to reduce and eliminate biofilm in in vitro and in vivo models. Contributions to practice it was found that clinical evaluation of the lesion does not allow the identification of biofilm, even when present; it is below the surface of the lesion. This finding raises reflection on the part of nurses regarding the interventions adopted for the removal of biofilm.

La cura avanzada de las heridas complejas se conoce desde 1994, por las guías internacionales para el manejo avanzado de las heridas. Las mismas han demostrado ser costo-efectivas contra la cura tradicional. Aun así, hoy en día, ¿se sigue curando de la manera tradicional las heridas o realmente se realiza la curación avanzada? Se logró determinar cuánto conocen sobre el tema y cuánto lo aplican, enfermeras y médicos de servicios quirúrgicos. Con una simple encuesta, el personal de diferentes servicios quirúrgicos contestó y se observó que un 83% sabe esta alternativa para curar una herida compleja, pero solo un 67% conoce realmente el método. También se vio que un 53% la aplica y el 47% no lo hace por falta de medios, insumos o desconocimiento del método.


The advanced treatment of complex wounds has been known since 1994 by international guidelines for the advanced management of wounds. They have proven to be cost-effective against the traditional cure. Yet, today, do wounds continue to be healed in the traditional way or is advanced healing actually done? We were able to determine how much they know about the subject and how much they apply it, nurses and surgical doctors. With a simple survey, the staff of different surgical services answered and it was observed that 83% know this alternative to heal a complex wound, but only 67% really know the method. It was also found that 53% applied it and 47% did not because of lack of means, inputs or ignorance of the method

ABSTRACT

El objetivo de este trabajo es normatizar el uso de la terapia de presión negativa de heridas complejas en neonatos. La iniciativa surge a partir de la negativa de nuestros proveedores locales de proporcionarnos el material del sistema VAC (cicatrización asistida por vacío), por temor a las posibles complicaciones por su uso en neonatos y escasa bibliografía al respecto. Se presentan 2 casos tratados en neonatos entre 2018 y 2021, y se acompaña de revisión bibliográfica encontrada, de diferente etiología. Con las conclusiones obtenidas, se pretende establecer al VAC como una herramienta terapéutica eficaz en neonatos, de igual manera que se aplica, en toda herida compleja que lo requiera, sin importar la edad del paciente


The goal of this work is to standardize the use of negative pressure therapy for complex wounds in newborns. The initiative arises from the refusal of our local suppliers to provide us with the VAC (vacuum-assisted healing) system material, for fear of possible complications from its use in newborns and little literature about it. There are 2 cases treated in neonates between 2018 and 2021, and it is accompanied by found bibliographic review of different etiology. With the conclusions obtained, it is intended to establish the VAC as an effective therapeutic tool in neonates, just as it is applied, in any complex wound that requires it, regardless of the age of the patient

ABSTRACT

The objective of this study was to investigate the effects of a gel formulation from the association of Plectranthus neochilus and Cnidoscolus quercifolius on tissue repair in cutaneous wounds in rats. A surgical wound was induced in 35 Wistar rats and treated according to group: G1 - commercial phytotherapeutic gel; G2 - Carbopol gel 1%; G3, 4, 5 - gel formulation from Boldo-gambá and Favela (FGBF) at 2.5%, 5%, and 10%, respectively. 1ml of the product was applied topically daily, for 14 days. Macroscopic evaluation of the wound showed inflammation, granulation, and epithelization in all groups. The FGBF 2.5% group showed greater angiogenic potential. There was a significant difference between the surgical area of the wounds treated with FGBF 2.5%, 5%, or 10% compared to the group with the commercial phytotherapeutic gel. On histomorphometry of the skin, there were reepithelization of the epidermis and superficial dermis, longitudinal collagen fibers, fibroblasts, and blood vessels, and in the deeper dermis, fibroblasts, transverse and longitudinal collagen fibers, blood vessels, and inflammatory cells. The 2.5% formulation had the greatest increase in fibroblast proliferation and most intense collagenization on day 14 of treatment.(AU)


Este trabalho objetivou investigar os efeitos de uma formulação em gel da associação da Plectranthus neochilus e da Cnidoscolus quercifolius no processo de reparação tecidual em feridas cutâneas de ratos. Foi induzida uma ferida cirúrgica em 35 ratos Wistar, sendo tratadas de acordo com os grupos: G1 - fitoterápico comercial; G2 - gel de carbopol 1%; G3, G4 e G5 - formulação gel boldo-gambá mais favela (FGBF) 2,5%, 5% e 10%, respectivamente. Aplicou-se 1mL do produto, via tópica, diariamente, durante 14 dias. Na avaliação macroscópica das feridas, verificou-se inflamação, granulação e epitelização em todos os grupos. O grupo FGBF 2,5% apresentou maior potencial angiogênico. Houve diferença significativa entre as áreas cirúrgicas das feridas tratadas com os FGBF 2,5%, 5% ou 10%, comparados ao grupo com o gel fitoterápico comercial. Na histomorfometria da pele, observou-se reepitelização da epiderme e da derme superficial, fibras colágenas longitudinais, fibroblastos e vasos sanguíneos e, na derme profunda, fibroblastos, fibras colágenas transversais e longitudinais, vasos sanguíneos e células inflamatórias. A formulação a 2,5% teve o maior aumento na proliferação de fibroblastos e mais intensa colagenização no dia 14 de tratamento.(AU)

Na procura por material alternativo no tratamento de feridas cutâneas, buscou-se com este estudo avaliar o comportamento da túnica vaginal canina conservada em glicerina a 98% como curativo biológico no tratamento de feridas cutâneas provocadas experimentalmente em ratos. Foram utilizados 16 ratos Wistar, nos quais foram induzidas experimentalmente duas lesões cutâneas na região dorsal, sendo uma cranial e a outra caudal. Uma ferida serviu de controle, a qual não recebeu tratamento, a outra recebeu a túnica vaginal recobrindo toda extensão da lesão. A seleção de qual das feridas, cranial ou caudal, iria receber a membrana foi feita de forma aleatória. Os animais foram distribuídos em quatro grupos de igual número para análises clínicas, macroscópicas e histológicas decorridos três, sete, 14 e 21 dias de pós-operatório. Na avaliação clínica e macroscópica pode-se observar que o processo de reparação teve evolução semelhante tanto no grupo controle quanto no grupo membrana. Na análise histopatológica foi constatada grande quantidade de infiltrado inflamatório nos períodos iniciais, no entanto, decorridos 21 dias, a epiderme já se encontrava reparada, e em todos os momentos foi observada maior presença de tecido conjuntivo no grupo membrana, sugerindo maior precocidade na cicatrização. Deste modo, concluiu-se que a túnica vaginal canina pode ser utilizada como nova alternativa no tratamento de feridas cutâneas.


In the search for alternative material in the treatment of cutaneous wounds, this study aimed to evaluate the behavior of canine vaginal tunic conserved in glycerin 98% as a biological dressing in the treatment of cutaneous wounds provoked experimentally in rats. Sixteen animals were used in which two cutaneous lesions were caused in the dorsal region, one cranial and the other caudal. One wound served as a control which did not receive treatment, the other received the vaginal tunic covering all extension of the lesion. The selection of which of the wounds, cranial or caudal, would receive the membrane was made at random. The animals were divided into four groups of equal numbers for clinical, macroscopic, and histological analyzes after three, seven, 14 and 21 postoperative days. In the clinical and macroscopic evaluation, it can be observed that the healing process had similar evolution in both the control group and the membrane group. In the histopathological analysis, a large amount of inflammatory infiltrate was observed in the initial periods; however, after 21 days the epidermis was already healed, and at all times a greater presence of connective tissue was observed in the membrane group, suggesting a greater precocity in healing. Thus, it was concluded that the canine vaginal tunic can be used as a new alternative in the treatment of cutaneous wounds.

Objetivo: caracterizar o conhecimento dos agentes comunitários de saúde sobre trauma de extremidades. Método: trata-se de um estudo quantitativo, descritivo, realizado a partir do banco de dados da pesquisa intitulada "conhecimento de agentes comunitários de saúde sobre atendimento pré-hospitalar no suporte básico de vida - impacto das ações de extensão". os dados foram coletados por meio de questionário. a amostra foi composta por 15 agentes comunitários de saúde de uma unidade de saúde da família integrada em João Pessoa, paraíba, brasil. analisaram-se os dados por meio de estatística descritiva e foram apresentados os resultados em tabelas. Resultados: observa-se que a média de acertos no pós-teste sobre conhecimento no trauma de extremidades foi de 69,3%. Conclusão: evidenciou-se que o conhecimento dos agentes comunitários de saúde sobre traumas de extremidades, teve um aumento significativo após as intervenções educativas

Resultados: se observó que el promedio de aciertos posteriores a la prueba en el conocimiento del trauma en las extremidades fue del 69,3%. Conclusión: se evidenizó que el conocimiento de los agentes de salud comunitarios sobre trauma de las extremidades, tuvo un aumento significativo después de las intervenciones educativas

RESUMO Objetivo realizar avaliação clínica e sociodemográfica de pacientes com úlceras de perna. Método estudo transversal, quantitativo, com 105 pacientes com úlceras de perna em ambulatórios da rede pública de Niterói/RJ, aprovado pelo Comitê de Ética em Pesquisa da Faculdade de Medicina da Universidade Federal Fluminense. Os dados foram coletados no período de janeiro a agosto de 2019, analisados por estatística descritiva e dispostos em tabelas. Resultados pacientes do sexo masculino (57,1%), de 60 a 80 anos (60%), com ensino fundamental incompleto (45,7%), renda de até 1 salário mínimo (64,8%). A maioria das lesões foi de etiologia venosa (76,2%), de tempo igual ou superior a 40 meses (54,3%), com tamanho maior que 10cm2 (53,3%), apresentando exsudato seroso (91,4%) em pequena quantidade (40%) e predominância de tecido granulado (36,2%). A dor foi um achado frequente, relacionada com a posição do membro (31,4%). Conclusão e implicações para a prática o conhecimento do perfil da população acompanhada e das características clínicas das lesões favorece a otimização do tratamento, permitindo um planejamento específico da assistência de enfermagem.

Los datos fueron recolectados de enero a agosto de 2019, analizados por estadística descriptiva y presentados en tablas. Resultados pacientes del sexo masculino (57.1%), de 60 a 80 años (60%), con educación primaria incompleta (45.7%), ingresos de hasta 1 salario mínimo (64.8%). La mayoría de las lesiones fueron de etiología venosa (76,2%), con una duración de 40 meses o más (54,3%), con un tamaño superior a 10 cm2 (53,3%), presentando exudado seroso (91,4%) en pequeña cantidad (40%) y predominio de tejido granulado (36,2%). El dolor fue un síntoma frecuente, relacionado con la posición del miembro (31,4%). Conclusión e implicaciones para la práctica el conocimiento del perfil de la población monitoreada y las características clínicas de las lesiones favorecen la optimización del tratamiento, lo que permite una planificación específica de la atención de enfermería.

Results most were male patients (57.1%), aged between 60-80 years (60%), with incomplete primary education (45.7%) and monthly income up to 1 minimum wage (64.8%). Most ulcers were of venous etiology (76.2%), present for 40 months or more (54.3%), with size greater than 10cm2 (53.3%), having serous exudate (91.4%) in small quantity (40%) and predominance of granulation tissue (36.2%). Pain was a frequent finding, related with the leg position (31.4%). Conclusion and implications for practice the knowledge of the population profile and clinical characteristics of wounds favors treatment optimization, allowing specific nursing care planning.

Throughout the past 2020, the pandemic COVID-19 has caused a big global shock, meanwhile it brought a great impact on the public health network. Trauma emergency system faced a giant challenge and how to manage trauma under the pandemic of COVID-19 was widely discussed. However, the trauma treatment of special population (geriatric patients and patients taking anticoagulant drugs) has received inadequate attention. Due to the high mortality following severe traumatic hemorrhage, hemostasis and trauma-induced coagulopathy are the important concerns in trauma treatment. Sepsis is another topic should not be ignored when we talking about trauma. COVID-19 itself is a special kind of sepsis, and it may even be called as serious systemic infection syndrome. Sepsis has been become a serious problem waiting to be solved urgently no matter in the fields of trauma, or in intensive care and infection, etc. This article reviewed the research progress in areas including trauma emergency care, trauma bleeding and coagulation, geriatric trauma and basic research of trauma within 2020.

ABSTRACT Objective To describe the knowledge translation obtained through research of low-level laser therapy to wound treatment in the clinical practice. Methods Experience report about implementation strategies of this new technology in a university hospital, being an extension course the main one, developed between May and December 2019. Results The course was carried out by researchers in partnership with clinical nurses to 13 nurses from different areas with previous experience in wound care. Were taught 30 theoretical-practical class hours, part of them supervised in a real clinical setting, besides case studies. From that, an exclusive nursing consultation schedule of laser therapy was created at the outpatient and its use was also improved to patients admitted in several units of the hospital. Final considerations The knowledge translation developed about laser therapy allowed its implementation as a technology in wound treatment, qualifying the professional practice and benefiting the treatment of patients with wounds.


RESUMEN Objetivo Describir la traslación del conocimiento obtenido en investigación de la terapia con láser de baja potencia para el tratamiento de heridas en la práctica clínica. Métodos Relato de experiencia sobre estrategias para la implementación de esta nueva tecnología en un hospital universitario, con un curso de extensión desarrollado entre mayo y diciembre/2019. Resultados El curso fue realizado por investigadores en alianza con enfermeras especializadas para 13 enfermeros de diferentes áreas y experiencias en el cuidado con heridas. Fueran dados 30 horas de clases teórico-prácticas supervisadas en un entorno clínico real y estudios de casos. En consecuencia de esto, se creó consultas de enfermería de terapia láser, así como un aumento de esta terapia para los pacientes hospitalizados. Consideraciones finales La transferencia de conocimiento sobre la terapia con láser permitió la implantación de esta tecnología en el tratamiento de heridas, con práctica profesional calificada y beneficio el tratamiento de pacientes con heridas.


RESUMO Objetivo Descrever a translação do conhecimento obtido em pesquisa sobre terapia a laser de baixa potência para o tratamento de feridas na prática clínica. Métodos Relato de experiência sobre estratégias de implementação dessa nova tecnologia em um hospital universitário, sendo um curso de extensão a principal delas, desenvolvido entre maio e dezembro/2019. Resultados O curso foi realizado por pesquisadores em parceria com enfermeiros assistenciais para 13 enfermeiros de diferentes áreas e experiências no cuidado a pacientes com feridas. Foram ministradas 30 horas de aulas teórico-práticas, parte das mesmas supervisionadas em cenário clínico real, além de estudos de caso. A partir disso, foi criada uma agenda de consulta de enfermagem de laserterapia no ambulatório, bem como incrementado essa terapia em pacientes hospitalizados. Considerações finais A translação do conhecimento sobre laserterapia permitiu a implantação dessa tecnologia no tratamento de feridas, qualificando a prática profissional e beneficiando o tratamento de pacientes com feridas.
